Dr. Crystal Gulotta
Dr. Crystal Gulotta DNP, CNM, FNP has been rooted in healthcare since 1993, and has spent her lifetime on endeavors of both wellness and illness services across multiple spectrums and has continued this lifetime achievement by now offering a concierge personal health clinic experience to those who become clients of her and her company at A Plus Concierge NP, a division of A Plus NP Care, LLC. This company will provide Wellness, Weight Loss, Hormones and Rapid Care with custom options with in each.
Crystal is a Doctoral Educated Advanced Practice Registered Nurse ( APRN) and she has advanced practice provider experience in urgent cares, research , as well as high and how risk pregnancy. She launched into this wellness , weight loss and hormone space with the strong belief that Wellness and feeling better with Hormones and Healthy Weight is the way to a more joyful optimal health experience as we age, as well as detour the amount of Rapid Care illness’s and chronic illness a person experiences. She envisions this space to be accessible, customizable and personal. She and her team are here to care for clients in sickness, and in health.
Education and Training: Dr. Gulotta completed her Doctorate of Nursing Practice ( DNP) and Certified Nurse Midwife (CNM) education through the highly respected Frontier Nursing University, followed by the completion of a Family Nurse Practitioner (FNP) Post Masters Certificate with South University. Prior to the advanced practice education, she completed undergraduate nursing degrees with Pima Community college in Tucson, Arizona and University of Phoenix. She has obtained multiple state certifications as a Family Nurse Practitioner. Education and training for specialty services as an advanced practice provider is ongoing and has thus far focused on sexual health, male hormones, weight loss , women health, pregnancy, trauma, pediatrics, primary care and more. Continuing education is ongoing as required by law and encompasses all of the above as well as meeting all required certifications of each license.
Crystal Is joyful to share that she is married to John, her husband and a father to their four grown children; three sons and one daughter. Together they have four grandchildren, three girls and one boy. The pitter pawter of three dogs who enjoy the good life at their home completes the family. She enjoys the simple things in life like reading, great foods, dancing, listening to music and believe it or not, constantly reading up on health avenues. She believes God has led her down this path of opening this business, and that his hand is guiding her. She chose a butterfly as the symbol for the company because it is a transformation symbol of how no matter where you start, good things come. She believes you can start today, learn more, do more, look forward, be your best self, and transform to who you know yourself to be.

A + Concierge NP is a private concierge health clinic physically located in Tucson, Arizona, and we do basic male and female hormone optimization in Arizona with in person visits by appointment as needed, with telehealth as our primary monthly service visit type. We also offer medically assisted weight loss with oral and injectable medications, and basic rapid care services for non life threatening illness by telehealth in Arizona, Alaska, Montana, Florida, and Washington. Our services are not contracted with insurance companies.
Our wellness service is focused on making quality supplements available, making Lifewave phototherapy patches available and making labs more available with our LabShop all of which can be found on the Shop page. We Look forward to helping provide you with a concierge level service with a personalized approach to help you achieve your health goals.
